Assuming the King is, well, a King and not a crazy monster or something, i would assume he chooses who gets to join the Royal Guard, ie, King->Royal Guard->Central 46->Yamamoto

That makes sense. I wonder why Captains like Shunsui and Juushirou were never promoted? Probably because Shunsui's lazy and Juushirou's illness.

They need some experienced Captains to stay in the Gotei 13, and they are very experienced. Otherwise, Captains would probably be getting promoted left and right. Yama needs to stay because he's the Commander. Beyond that, you just can't replace Shunsui, Ukitake, and Unohana very easily. They also most likely have a choice as to whether or not they wish to join the Royal Guard or not.

Also, Kings/Queens usually don't do anything, they have other people do it for them. So more than likely there's a trusted adviser of some sort that picks out who is worthy enough to join the Royal Guard, and submits requests for the promotion?

From the way they made it sound with Hikifune, you can decline you acceptance in to the Royal Guard. So it's possible that captains like Shunsui, Ukitake and Unohana just declined.

i think shunsui and ukitake are just too loyal to yama-jii, so they dont wanna join the royal guard.

Well, that was kind of a big deal in itself.

Even then, they were all reluctant to fight each other.

Yama probably thinks of Shunsui and Ukitake as his sons. Those two probably think of him as their father, in a sense. I doubt those two would leave him to be in the Royal Guard.
