Ichigo sympathized with Grimmjow regarding the dream that he chased for so long. I'm fairly sure he's going to give the okay to Orihime for the heal. Plus, she's already healed him once and pretty much owes him for the breakout of her cell (along with the defeat of her tormentors at the time).

He's completely drained, though. Both of them are. I doubt Noitora cares if he fights an enemy at full power or not. He just has that "MUST KILL" gleam in his eyes. Just about always.

what I'm guessing will happen is ichigo will try to fight him but wont be able to use his mask, ichigo will be getting his arse kicked , orihime will realize that she wants to protect ichigo with her powers, and she defeats noitora, then ulquirra shows up and the back up comes in and saves ichigo and orihime

Ulquiorra is obedient, theres nothing we can do about that. He follows orders ruthlessly, the only case where he didn't was when he was ordered to kill Ichigo when they first went down to the human world.

As for grimmy, he just has a craving to get stronger and to be the best, he is always looking for a fight. Probably it's cuz of a messed up life while he was alive.

Even if Ichigo manages to go Bankai Vaizard for another prolonged period of time, I doubt he's going to stand up to Noitora.

Either way, Ichigo will have to get out of Hueco Mundo. With or without his companions. My bet is that a "rescue" team will show up and pick up the remainings of Chad, Renji, Ishida and Rukia and dust off with Ichigo. Could be some of the captains (or vice captains and higher ranked seated officers) or the Vaizards.
And Orihime will stay in Hueco Mundo, be it because she refuses to escape or Aizen (or some of his underlings) prevent her from escaping.
